Job Summary:
Under the direct supervision of the Clinical Team Lead, the Patient Navigator–Community Health Worker plays a key role in supporting clients as they navigate behavioral health, physical health, and social service systems. The Patient Navigator will focus on patient and community engagement, outreach, care coordination, and advocacy. Ensuring individuals receive timely, culturally responsive and person-centered services. The Patient Navigator will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.

- High school diploma or equivalent
- 3 years of direct patient care experience.
- Community Health Worker Certification or be able to obtain within first three to six months of employment.
- Medical Assistant Certification, preferred.
- Experience working with diverse populations, including individuals with behavioral health or substance use needs, preferred.
