Job Summary:
The CCBHC Patient Specialist provides a full range of professional nursing services, including assessment, planning, implementation, evaluation, and coordination of patient care in accordance with individualized, team-based Comprehensive Care Plans. Under the direct supervision of the CCBHC Clinical Team Lead, the CCBHC Patient Specialist plays a critical role in supporting individuals as they navigate behavioral health, physical health, and social service systems. The CCBHC Patient Specialist will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.

  • Fluent written and verbal skills in English and Spanish strongly preferred

Job Requirements:
  • Bachelor’s degree in health-related field. Nursing or Social work degree preferred.
  • Minimum one (1) years’ experience in clinical care setting
  • Community Health Worker Certification or be able to obtain within first three months to six months of employment.
  • Experience working with diverse populations, including individuals with behavioral health or substance use needs, preferred
