Job Summary:
Under the immediate supervision of the Director of Administration is responsible for monitors and coordinates the activities of the assigned Patient Management Team, in partnership with assigned Clinical Supervisor, in compliance with established utilization of services and equipment.

Knowledge of and experience with the poverty level population and life-styles.
Job Requirements:
Associates degree (min. 2 years college) plus one year experience in similar position or high school diploma (or equivalent) plus two years experience in similar position. Must have minimum one year experience in a supervisory role.
The ability to consistently establish positive working relationships, communicate and work cooperatively with clients, staff, colleagues, and community members.
A thorough knowledge and use of community resources, Agency services, and requirements of funding systems.
